I am using OS X 10.1.3.
I downloaded both the .sit and .zip version of these files. Using Frontier 7.0.1 (with fully updated root), placing the formz.root in either of your recommended locations resulted in Frontier seeing a "faded" icon, and being unable to open the file. Double-clicking the file results in it opening in TextEdit. Dragging it onto the Frontier app icon results in it opening as a text file. I'm intrigued by the description of this tool, but stymied in the use of it!
